The athletic department is proud to partner with Dr. Forsyth and his staff to provide sports physicals to our student athletes.  Dr. Forsyth and his staff at Ascension St. Mary's will offer sports physicals for $15 on Friday, June 24th from 8:00 AM to 12:00 PM at Ascension St. Mary's, 1600 W Broad St, Chesaning, MI.

Please have the attached forms competed prior to arriving at the facility.  If you are unable to print the forms they are also available for pick up in the High School office, Monday through Friday from 7:00 AM to 12:00 PM.
