2024-25 SPORTS PHYSICALS

ATTENTION 2024-25 HIGH SCHOOL & MIDDLE SCHOOL (7th & 8th) STUDENT ATHLETES!! Sports physicals are required for all students that plan to play a school-sponsored sport in the upcoming 2024-25 school year. A walk-in sports physical clinic will be offered as follows:FRIDAY, JUNE 14th & FRIDAY, AUGUST 9th8:00 AM - 12:00 PM.Ascension St. Mary’s - Dr. Forsyth’s Office1600 Brady St., ChesaningCost: $15.00 (cash or check)Please make sure to have the MHSAA physical form with you when you get your physical.
